Tuner would be my choice the other things may help but tuning the computer to run better will for sure help.

Nothing wrong with the K&N or S&B.

I have the Vararam and it works too.

CAI's are hard to tell any difference between.

if its anything like mine not good ....at best 11mpg city 15.5/16 highway , cai,exhaust,trinity , 10" lift 35's ....hes got 37's so probably worse lol

To add to this, the Vararam has shown gains of MPG (especially on the highway) for many members here including myself.
I have seen an increase of 1-1.5MPG just on the freeway

As for gears, 4.56 would be too much for freeway driving and you will suffer MPG wise. I know as I ran it through winter. City driving is a different story.
Best gear option would be 4.10 if moving from 3.55
But if you have 3.92, that suffices and adding a tuner and other small bolt ons will better it for you

I just had 4.56 gears put in, had 3.55, also running 35 in tires. Mostly city driving. On the highway turning around 2200rpm at 60 mph. Didn't really notice much change in mpg. Around 12. My next mod will be a turner.

I have an S&B and i didn't notice any MPG gain, but I firmly believe that CAI don't really do much for MPG (definitely won't make it worse though). But some people do claim that their MPG does slightly go up. It did make the exhaust tone a little deeper.

Mod your exhaust. You don't have to do anything crazy, just remove the resonators and change out the muffler. I went with a Flowmaster Super 10, Really loud when you stomp on it and quiet when MDS kicks in. That's all you really need to do if you just want a better sound to your truck. Lifted trucks without a deep growl are silly to me.

I would go with 4.10's.

Answer to your question on which to do first.... It all depends on your wallet, but all except the throttle body spacer should be done.

Really? no MPG change?
I too have 4.56 and ran 35's through winter and then into early spring and when I swapped over to 37's, now Im turning at 1980 at 60mph and my MPG did increase.
Another advantage is...I dont have my exhaust buzzing in my ear anymore at 2200 rpm

For anyone running 35's and wanting to do a gear swap, I also recommend 4.10's for daily driving but 4.56 is surely fun on 35's

Yes, 4.56 is a lot of fun with the 35s. I only drive around 8000 miles a year. Most definitely keep you in mind when it comes time for a tuner. I just have to turn up the radio a little more on the highway. lol
